Volunteer hours log (PDF): 16 entries a page with the date, organization, what you did, hours, supervisor and signature, a page total, and student and parent signature lines. Print as many pages as you need.

Community service hours verification form (PDF): one page for the supervisor to list the dates and hours, sign, and confirm the service was unpaid.

What to write down, every time

  1. Date and hours, the same day you serve.
  2. Organization and what you did, in a few words.
  3. Supervisor: name and phone or email, and a signature.
  4. Proof: a photo, sign-in sheet or certificate, kept with the log.

Logging the same day matters: students who rebuild their hours from memory before a deadline usually undercount them, or cannot get a supervisor to confirm months later.

Logs for specific requirements

Some programs have their own rules. These guides each come with a matching printable log:

Or keep the log online, free

My Award Tracker is the same log on your phone: add an activity in about 30 seconds, attach a photo, and send your supervisor a one-click confirmation by email. Your totals and goals are always up to date, and the record is yours if you change schools. Logging and supervisor confirmation are free.

Frequently asked questions

What should a volunteer hours log include?

For every activity: the date, the organization, what you did, the hours, and a supervisor who can confirm it (name, phone or email, and a signature). Add a photo, sign-in sheet or certificate when you can.

Who should sign my volunteer hours?

The adult who supervised you at the organization, such as the volunteer coordinator. Most schools and programs do not accept a parent or relative as the supervisor.

Can I use this log for NHS, graduation or scholarships?

Yes, as your own record. Many schools and programs also have their own form; copy your entries onto it when they require it, and keep this log as your backup.

Is there a digital version?

Yes. My Award Tracker is a free online log: add an activity in about 30 seconds, attach a photo, and your supervisor confirms the hours with one click by email.
